SOURCE

Service Options Using Resources in a Community Environment (SOURCE) provides long-term health services in a person's home or community to improve the lives of persons with chronic conditions. This is accomplished through a framework of managed care to the consumer across all lines of services, diagnosis or disability.

Services available include personal care, adult day care, caregiver respite and home-making, home-delivered meals, emergency response and home health.

Eligibility and Costs

All SOURCE enrollees must:

Have SSI Medicaid or Public Law Medicaid
Be 65 or older, with a chronic medical condition OR under 65 and disabled
Meet the medical and functional criteria for placement in a nursing home

There are no out-of-pocket costs to seniors who are eligible for this program. Medicaid pays all costs.
